Chronic diseases, such as asthma, cancer, diabetes, and heart disease, are the #1 cause of death and disability in the U.S. They affect the quality of life of 133 million Americans and are responsible for seven out of every 10 deaths in the U.S. – killing more than 1.7 million Americans every year, according to the CDC.

The good news is that experts believe the vast majority of the death and disability caused by chronic disease is preventable or could be alleviated through better management of disease – if only our nation took the right steps.

Who We Are

The PFCD is a national coalition committed to raising awareness of the policies and practices that improve health and reduce health costs through more effective prevention and management of chronic disease.
